Timberlake Animal Hospital offers patient form(s) online so you can complete them in the convenience of your own home or office.

- Fax us your printed and completed form(s) or bring it with you to your appointment.
Please note: all new clients are required to put down a $45 deposit when making a first appointment at our clinic. This will be credited toward the cost of the first exam and will reserve your appointment slot. This is only required for the first appointment.
Please be advised: a $60 fee will be applied to your account if you fail to show for your appointment and do not call at least 12 hours ahead to cancel!
