Appointment Request

Use this form to request a wellness appointment with us. FOR SICK ANIMALS, please email us directly at [email protected]. While we will do our best to accommodate your requested day and time, please note, your appointment is not fully booked until you get a confirmation from us! We will contact you once we receive the form and let you know when your appointment will be. Please allow 1-2 business days for a response. 

Once your appointment is confirmed and booked, please be aware that there is a $35 fee that will be applied to your account if you fail to show for your appointment and do not call at least 2 hours ahead to cancel!

Please note: all new clients are required to put down a $35 deposit when making a first appointment at our clinic. This will be credited toward the cost of the first exam and will reserve your appointment slot. This is only required for the first appointment.
